糖尿病合并心血管疾病怎么办?控制三高是关键!

无论是1型糖尿病还是II型糖尿病,只要血糖持续处于高水平状态,都会毒害组织,促进心血管病发生。不少II型糖尿病患者伴有肥胖症,常常存在胰岛素抵抗,此类患者胰岛素并不缺乏,但身体处于高胰岛素血症状态,促进动脉粥样硬化生成,从而诱发且加重心血管病。
面对血糖和心血管疾病同时出问题,该如何处理?一起来看看吧!
一、糖尿病和心血管疾病之间有怎样的关联?
如果患有糖尿病,首先它会造成患者血管内膜的损伤。之后,血液中的血脂成分尤其是胆固醇,会通过损伤的血管内膜进入血管壁,在里面形成动脉粥样硬化斑块。这种斑块会越长越大,使血管出现狭窄甚至闭塞,从而造成冠心病、心肌梗塞的发生。
如果动脉粥样硬化斑块在大脑的血管中出现,就会造成脑血管病,甚至脑卒中、脑出血。如果出现在外周血管,也就是四肢的血管中,就会引发肢体的缺血或坏死,如糖尿病足。
二、糖尿病合并心血管疾病该如何管理?
1、管理好血压
糖尿病合并心血管疾病患者,应把血压控制在130/80mmHg,依据年龄、肝肾功能和病情严重程度选择适合的降压药,把血压控制在正常范围内。
采取低盐饮食,每天的吃盐量不能超过6g,远离加工食品。
2、管理好血脂
依据动脉粥样硬化性心血管疾病分类,把危险分为三种:
第一种是中危型,20~39岁糖尿病患者合并三高、吸烟酗酒和肥胖、视网膜病变、肝肾功能损害、蛋白尿以及左心室肥厚等。这种情况下应把坏胆固醇控制在2.6mmol/L;
第二种是高危风险,也就是年龄超过40岁的糖尿病,此时应把坏胆固醇控制在1.8mmol/L;
第三种是极高危风险,糖尿病合并已确诊的动脉粥样硬化性心血管疾病,此类人群应把低密度脂蛋白胆固醇控制在1.4mmol/L。平时应积极控制饮食,远离高胆固醇食物,戒烟戒酒,保持适度运动来维持正常体重,必要时服用他汀类药物。
3、管理好血糖
糖尿病患者,勤监测血糖和糖化血红蛋白来判断血糖情况,同时也要积极预防低血糖。
未满65岁、糖尿病病程不足10年、没有严重的心血管疾病患者,应把糖化血红蛋白控制在7%以下;糖尿病病程超过10年,出现严重的心血管疾病时,糖化血红蛋白控制目标在7%~8%;若年龄超过75岁,病程超过10年,应把糖化血红蛋白控制在8%~9%。

糖尿病患者应管理好饮食,少吃精制碳水化合物(主食)和脂肪类食物,多吃蔬果。每天有30分钟以上的中等强度运动,一周坚持4~5次。
严格按照医生嘱咐服用降糖药物,定期监测血糖,每三个月复查一次糖化血红蛋白。此外,保持规律作息,拒绝熬夜,以乐观心态面对病情,不要太焦虑。
总而言之,管住嘴、迈开腿、多运动,戒掉抽烟、喝酒等不良生活习惯,都能有效预防心血管疾病的发生。

影片“如此打工三十年”长约11分钟,在影音平台B站上,网易新闻发布的影片已经遭删,不过仍有不少网友接力转发在自己的账号下。
安徽省合肥市有3大民工集散中心,在影片拍摄的其中一处路口,一个上午就有约1000名外地来的农民工想找工作,大桥下也满是等著做日结(当天结算工资)工的人。
他们年龄偏大,多在55岁以上,做的是运垃圾、拆墙等劳力工作,而工地按规定基本上不收55岁以上的人。中国当前经济成长放缓、房地产不振,中高龄劳力谋生更加不利。
57岁的董桂兰,进城务工7年,她凌晨4点就到路口等待,看有没有活可干。她身体不适但不愿就医,因为一检查就要花人民币600、700元(约新台币3000元),而她已经两个月欠下房租了。她说媳妇吵架时威胁要丢下她不管,但当初娶媳妇就花了20多万元。
54岁的王兆喜,额头尽是深刻的皱纹。他到合肥10几年了,一早6时就到工地干活,晚上7时回家,虽然称自己已经累得搬不了砖,但又说还得再做10年。因为,他有一子、一女、一孙子,都仰赖着他。
49岁的汪先生,进城务工已经33年,他说自己是装修工人,但扛垃圾、杂活都做。他展示手机里收到村里发来催缴医保的简讯,说自己一直找不到工作,所以交不起这380元。
中国的新型农村合作医疗2003年建立时,个人缴费标准仅为10元,到了2023年已经是380元。媒体报导指出,去年不少人主动退保,很多农村居民反映“保不起”或是觉得“不划算”。
影片也呈现出辛酸的场面,有些人为了争工作机会起冲突扭打,有人被坑骗,奔波一趟后发现白忙一场。
在街边提供免费午餐的陈先生说,越来越多人来排队领免费午餐,最多时有200多人。他问一些已经65岁、68岁的农民工,为什么还要出来工作,得到的回复是想要赚点钱给老伴养老,因为孩子也有孩子的压力。
影片说,这一代的农民工“从小吃不饱饭,到吃饱饭了就开始干苦力,干完苦力就开始打工,打完工就老了,老了还得干。盖了那么多房子,但是,没有一间属于他们自己的。”
根据国家统计局,2022年中国农民工总量2亿9562万人。
自媒体、微信公众号“城市的地得”今天发文指出,江苏北部、安徽北部、河南东部一带,高龄农民工的处境就是如影片内容。
文章说,他们从90年代初开始进城务工,至今差不多正好30年。这是城市化和房地产大发展的30年,他们亲手建造起了一个个城市,现在他们老了,只能在城乡结合部的劳动市场游荡。
作者呼吁,如果国家每年能拿出2000亿元,全部用来补贴最低收入者,就能够很大程度上改善他们的处境。“不要信胡锡进‘都发钱等于都没发’那种说辞。世界上大部分国家都是这么做的,补贴最低收入者,和通货膨胀没一分钱关系。”
